Description
The Individual Performance Rating Template provides an opportunity for the immediate supervisor to prepare the form for a subordinate person and rate their performance based on their role and specific position checklist criteria. The rating is reviewed with the individual who signs and dates the form which is then forwarded to the Planning Officer before the individual leaves the incident.
